Background Screening Disclosure and Authorization
DISCLOSURE
Ridgeway Plumbing, Inc. ("the Company") may obtain a consumer report and/or investigative consumer report about you for employment-related purposes, to extend permitted by applicable federal, state, and local law.
Such reports may include information regarding your criminal history, employment history, education, professional licenses or certifications, driving-related information where job-related, and other background information obtained from consumer reporting agencies, public records, former employers, educational institutions, or other lawful sources.
An investigative consumer report, if obtained, may include information gathered through personal interviews regarding your character, general reputation, personal characteristics, or mode of living.
You have the right to request additional information about the nature and scope of any investigative consumer report obtained.

AUTHORIZATION
I hereby authorize Ridgeway Plumbing, Inc. and its authorized agents to obtain consumer reports and/or investigative consumer reports about me for employment purposes at any time after receipt of this authorization, including after a conditional offer of employment, where permitted by law.
I understand that refusal to provide this authorization may impact my eligibility for employment.

Motor Vehicle Record Disclosure and Authorization
DISCLOSURE
Ridgeway Plumbing, Inc. ("the Company") may obtain a Motor Vehicle Record(MVR) about you for employment purposes when driving is a job-related requirement.
The MVR may include information related to your driver's license status, restrictions, violations, suspensions, revocations, accident history, or other records maintained by state motor vehicle agencies.
If you are hired into a position that requires driving, the Company may obtain updated MVRs periodically during your employment, consistent with business necessity and applicable law.
AUTHORIZATION
I hereby authorize Ridgeway Plumbing, Inc. and its authorized agents to obtain my Motor Vehicle Record from the appropriate state motor vehicle agency, including but not limited to the Florida Department of Highway Safety and Motor Vehicles, and any other state agency that maintains such records.
I understand that refusal to provide this authorization may disqualify me from consideration for positions that require driving as an essential job function.

I also understand and agree that no representative of the company has any authority to enter into any agreement for employment for any specified period, or to make any agreement contrary to the foregoing unless it is in writing and signed by an authorized company representative.
